We're so grateful that you want to join us on Sunday, August 9 at 9:o0am to pack backpacks with Temple Judea's Social Action Committee.
Every year, the Social Action Committee purchases and packs hundreds of backpacks and sends them to School on Wheels, to benefit students all over Los Angeles County who experience homelessness. This year, where the future of schooling is so uncertain due to COVID-19, supporting the tutoring and mentoring work of School on Wheels is more important than ever.
In accordance with Temple Judea policy regarding safety precautions and COVID-19, please note the following policies for this event:
- All participants must be able to (i.e. be old enough to):
- maintain 6' of physical distance from other non-related participants
- wear mask and gloves (provide your own) for the duration of the event
- follow instructions of staff and committee leaders
- Prior to entering the building, all participants will:
- Provide contact information for contact tracing (on this form)
- Sign liability waiver (one adult can sign this form, all others can sign a physical copy in-person)
- Submit to a temperature check

- There will be a registration cap to ensure that we have no more than 50 people. The event will be outside and it may be hot, so please bring your own water and/or sunscreen.
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Association. COVID-19 is reported to be extremely contagious. The state of medical knowledge is evolving, but the virus is believed to be spread from person-to-person contact and/or by contact with contaminated surfaces and objects, and even possibly in the air. The exact methods of spread ad contraction are unknown, and there is no known treatment, cure or vaccine for COVID-19. COVID-19 can cause serious and potentially life threatening illness and even death.
Tempe Judea cannot prevent you or your child(ren) from being exposed to, contracting, or spreading COVID-19 while on our premises. It is not possible to prevent against the presence of the disease. Therefore, if you choose to attend events or programs at Temple Judea, you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.
ASSUMPTION OF RISK. I have read and understood this warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 for myself and/or my children, including, if I am pregnant, my unborn child(ren), in order to utilize Temple Judea's services and enter its premises.
WAIVER OF LAWSUIT/LIABILITY. I hereby forever release and waive my right to bring suit against Temple Judea and its owners, officers, directors, officials, trustees, agents, employees, or other representatives in connection with exposure, infection, and/or spread of COVID-19 related to utilizing Temple Judea's services and premises. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.
CHOICE OF LAW. I understand and agree that the law of the State of California will apply to this waiver.
I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE, AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE.
