Important Information :

• EVERY SINGLE PERSON getting tested needs to have a filled out form before entering the parking lot, every time you test. Click HERE for the form.
• Testing results are sent through email. Please be sure to write your email address on the form legibly.
• IF THIS IS YOUR FIRST TIME TESTING WITH US, YOU WILL NEED THE ITEMS LISTED BELOW IN ADDITION TO THE FORM ABOVE.
• EVERY SINGLE person getting tested needs, printed front and back copy of their Insurance Card attached to their form, before entering the parking lot. (CHILDREN NEED PARENT'S CARD ATTACHED IF THEY DON'T HAVE THEIR OWN)
• EVERY SINGLE person getting tested with a Drivers License, also needs a printed copy of that attached to their paperwork before entering the parking lot.
• If you do not have the completed testing form, printed copy of insurance card, and printed copy of ID, you will not be allowed to test.  Please come prepared.  
• Please register the total number of people in your family who will be tested in the drop down menu below. We are limited to 100 tests per hour, your headcount is not optional.
